Q: Will my personal information be kept confidential?
A: Information will be used ONLY to process and verify names to be
engraved on the monument Wall of Honor. Your information will be kept
completely confidential.
Q: Who is eligible to be listed on the monument?
A: Any donor who has a Utah connectoin may be engraved on the monument. All donors of organs, eyes, tissue, marrow,living kidney, liver donors, whole body donations, and supre blood donors (10 gallons or more) are eligible.
Q: Does it cost money?
A: There is absolutely no charge for any donor to be engraved on the
monument. It is a gift to donor heroes for the gift of life given to another.
Q: If I sign this form does it make me a donor?
A: The Name Submission Form is to be used ONLY to submit a name to
be engraved on the Celebration of Life Monument. It is NOT a donor registry
form. To sign-up to become an organ, eye, and tissue donor or to learn
more about donation, please go to www.Yesutah.org or call 1-866-YES-UTAH
Q: How do I fill out the form?
A: If you are submitting the name of a loved one (a deceased donor)
follow the instructions and fill in every line on the right side of the
form under "for deceased donors", if you are a living donor) follow the
instructions and fill in every line on the left side of the form under
"for living donors". All submissions must be signed, dated, and notarized.
Q: We donated my son's organs, eyes, and tissue when he died in another
state. Can his name be engraved on the monument?
A: YES! Anyone who has a Utah connection may be engraved on the monument.
Please submit his name so that he may be honored as a hero.
Q: Why are there two forms?
A: The left-hand side of the form is for living donors; the right-hand
side is for deceased donors. Please complete the applicable side. All
submissions must be signed, dated, and notarized.
Q: My husband, mother, and son were all donors; may I submit more than
one name?
A: Yes! Use one form for each donor. You may download form from this
website, or call 562-1575 for additional forms.
Q: May I submit a quote on my submission form?
A: No, Only donor names can be submitted for the monument. The five quotes
that appear on the monument are from selected community leaders.
Q: Can I fax my submission form or do I need to send the original form?
A: No, we need an original, notarized copy of the form to process your
name submission.
Q: Can I use both sides of the form for deceased donor or do I need
to get another form?
A: You must submit only one person per form.
Q: My loved one has two last names, Can I put both names on the form?
A: Yes, you may list the name in any way you choose.
