Donation is about LIFE. Those who have chosen donation are true heroes. They have selflessly passed along the gift of life. The Celebration of Life Monument is being built as an enduring tribute to donors and their families for their heroic acts. There are many different types of donors.

If your loved one was an organ, eye, tissue, or whole body donor upon death, his or her name may be engraved on the monument.


Click the Deceased Heroes button above to confirm or review the name of each deceased hero, as it will be engraved on the Wall(s) of Honor. Click Living Heroes to confirm or review the name of each living hero.

If you are confirming the spelling or completion of a name submission, please e-mail your approval by clicking here and entering the name you are confirming on the subject line. A simple message of "confirmed" or "approved" will complete your submission. For changes or corrections, please call (801) 840-1773.

If you are a living donor...kidney, liver, bone marrow or super blood donor (10 gallons or more), your name may also be engraved on the monument.

This is a state monument; every donor who has any connection to Utah may have his or her name engraved on the monument Wall of Honor free of charge.
